We follow up with the family

We contact the patient or family directly — same day in most cases — to introduce ourselves and schedule an in-home assessment.

Assessment & care plan

One of our Operating Directors meets with the family in person, builds a care plan, and introduces the matched caregiver.

Care begins — we close the loop

Care starts, typically within 24 hours of assessment. We notify the referring professional that care is in place and provide a point of contact for any questions.

Serv's non-medical care frequently runs alongside skilled home health, hospice, or other clinical services. We coordinate openly with other providers and do not overlap with clinical scope. When clinical needs arise, we communicate proactively to the appropriate team.
